The Office of Child Care Licensing (OCCL) received a complaint on May 1, 2025, alleging improper discipline, failure to provide truthful information to parents about their child's daily experience, and medication administration without proper documentation. An investigation was conducted beginning May 7, 2025, and concluding May 9, 2025, which included a review of medication records, staff and administrator interviews, program observations, an interview with the Reporter, and review of four hours of audio recordings. The allegation of improper discipline is substantiated, based on the audio recording that captured inappropriate tones, threats, and unkind language directed at children. There was no evidence to substantiate a staff member swinging a child by the arm. The allegation regarding medication administration without proper documentation is substantiated, as the MAR for Child #1 indicated daily administration throughout February, yet Parent #1 received the inhaler unused. There is no evidence to substantiate this complaint for providing false information about a child s feedings or day based on interviews with staff and observation of daily records. While the facility s method of completing the daily logs could be confusing for a parent, they are not providing false information. An additional concern noted during this investigation was a of positive interactions with children in care based on the audio recordings.
