OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. Between July 30, 2025, and August 4, 2025, the Office of Child Care Licensing received three separate complaints regarding failing to use appropriate behavior supports. An investigation began on August 8, 2025, and was concluded on September 15, 2025. After reviewing video footage and conducting interviews, this complaint is substantiated for failing to use appropriate behavior supports. All staff members interviewed stated that they did not witness Staff #1 strike Child #1. The video footage did not provide any indication that Child #1 was hit by Staff #1. However, video footage reviewed by LS #1 documented a harsh tone was used by Staff #1 when addressing Child #1. The video footage captures Child #1 approaching Staff #1 to show a drawing. In response, Staff #1 dismissed Child #1 using a harsh tone, saying, Go sit down! Administrator #1 stated they wrote up Staff #1 for the tone they used when speaking to Child #1. Another staff member stated Staff #1 can use a harsh tone toward the children.

The Office of Child Care Licensing (OCCL) received a complaint on February 28, 2023, alleging improper staff-to-child ratios and exceeding maximum group size. An investigation was conducted on March 6, 2023. The complaint is substantiated for improper staff-to-child ratios because during the visit the preschool room contained 10 children, the youngest of which was two years old, supervised by one qualified staff person. According to two staff, the infant room had two infants in the morning, who were brought to the toddler room so the person supervising them, Staff #3, could cook and serve lunches. The two staff stated Staff #3, who has no DEEDS certification, would be taking the two infants back to the infant room after lunches were served. There is no evidence to substantiate this complaint for exceeding maximum group size, based on observations, interviews of staff, and a review of attendance documentation. Additional concerns noted were during the visit there was not enough staff present to operate, toddlers were not cared for in an area separate from the older children, staff stated an infant had been sleeping in a cradle/swing, instead of a crib or pack and play, children and staff attendance records were inaccurate, and did not correctly document the location of individual children and staff.

The Office of Child Care Licensing (OCCL) received a complaint on November 2, 2022, regarding a lack of supervision and not responding appropriately to a medical emergency. An investigation was conducted on November 9, 2022. Based on the results of this investigation this complaint is substantiated because the Reporter provided the specialist with an accident/injury report which indicated Child #1 had a seizure while in care and also discharge paperwork from the child s doctor which states the child possibly suffered a seizure while in care. There is no evidence to substantiate a lack of supervision because both staff members who were interviewed stated they have never observed bullying taking place at the center.

The Office of Child Care Licensing (OCCL) received a complaint on September 21, 2022, regarding improper discipline, improper staff-to-child ratios, mixed ages of children at incorrect times, not following applicable federal, State, and local laws and regulations, unqualified staff alone with children, the administrator not being present at least 50 percent of the hours of operation, and food policy violations. An investigation began on September 28, 2022, and ended on October 24, 2022. The complaint is substantiated for mixed-age groups present at times not within the first and last 90 minutes of the hours of operation, as documented on the attendance records sent to Ms. Catka. The complaint is substantiated for not following applicable federal, State, and local laws and regulations because the Wilmington Fire Marshal s Office inspected the facility on October 24, 2022, and found several violations, including the bed and other furniture set up in the basement. An additional concern noted is Staff #2 and Staff #3 did not have a DEEDS certificate in their personnel files at the time of the visit. There is no evidence to substantiate this complaint for improper discipline because Staff #1 and Reverend Williams denied Staff #1 ever asked an older child to hit a younger child. There is no evidence to substantiate the complaint for improper staff-to-child ratios, as all attendance records showed classrooms to be in correct staff-to-child ratio. There is no evidence to substantiate this complaint for unqualified staff alone with children, as attendance records reviewed by Ms. Catka showed qualified staff with children at all times. There is no evidence to substantiate this complaint for the administrator not being present at least 50 percent of the hours of operation, based on documentation which was provided to OCCL. There is no evidence to substantiate this complaint for food policy violations because Reverend Williams denied giving her mother s food to children or children s food to her mother.

The Office of Child Care Licensing (OCCL) received a complaint on October 25, 2022, regarding the center not following meal service requirements, cleanliness and sanitation concerns, improper discipline, and not following center policy regarding photography of children and social media. An investigation was conducted on October 31, 2022. Based on the results of this investigation this complaint is substantiated for cleanliness and sanitation concerns because it was observed the carpet, rugs, and hardwood floors in the preschool room and the toddler room had stains, dirt, and debris. It was also observed the walls in the preschool rooms had peeling paint, dirt, a broken floor vent, and a missing baseboard. Other concerns noted at the visit were an infant was observed sleeping in a crib without a name label and not placed on the lowest mattress setting, and an unqualified staff member was observed alone in the infant classroom. There is no evidence to substantiate this complaint for food being prepared outside of the center, being provided by Meals on Wheels, or that the center has no posted menu because the specialists observed a posted menu at the time of the visit, evidence of meal preparation, and food was labeled with expiration dates. Also, there is no evidence to substantiate this complaint for improper discipline because the center administrator, Staff #1, and Staff #2 all stated they have never observed any staff member speaking to children harshly or using improper discipline with a child. Also, the center administrator, Staff #1, and Staff #2 all stated they had no knowledge of pictures or videos of children being posted to social media.

A complaint was received on June 9, 2022, alleging failure to notify a parent immediately of a serious or potentially serious injury, lack of supervision, and lack of respectful communication by staff. An investigation was conducted on June 15, 2022. There is no evidence to substantiate the lack of supervision, as Staff #1 stated Staff #2 witnessed Child #1 being hit by the ball. There is no evidence to substantiate lack or respectful communication, as Pastor Charles denied speaking unprofessionally to the Reporter. This complaint is substantiated for failure to immediately contact a parent or guardian after the child was hit in the face with the ball.

On November 12, 2021, a complaint was received alleging improper discipline of a child. An investigation was conducted on November 16, 2021. The complaint is substantiated for improper discipline, because Pastor Rhonda tried to move a child along in a way that did not encourage the child to gain control of her emotions, and caused the child to believe Pastor Rhonda had plucked her on the head, prompting the child to resist further.

On August 30, 2021, a complaint was received regarding possible over-feeding of infants, infants not having feeding statements and schedules, and the center not having a first aid kit. An investigation was conducted on September 3, 2021. The complaint is substantiated for not having feeding statements and schedules properly filled out and referred to by staff. There is no evidence to substantiate the allegation that infants were being over-fed. There is no evidence to substantiate the center not having a first aid kit, as it was seen on several previous visits to the center and on the day of the investigation visit. Other concerns noted were that Staff #1 was being left alone with children at times throughout the day, though Staff #1 was not deemed eligible and had not been fingerprinted until nearly a month after date of hire; mouse droppings were found on a lower shelf in the infant room; untruthful information was provided to the licensing specialist because the administrator reported she always worked in the infant classroom with a staff who was not yet determined eligible and that staff said the administrator does not ever work in the classroom and another staff stated the administrator was usually in the infant room unless she was doing work in her office or making lunch and checking on other classrooms; juice was being fed to an infant; and an electric outlet plate was missing in the infant room and the outlets themselves were not covered.
