On March 18, 2022, a complaint was called into OCCL regarding improper staff-to-child ratio and a staff at the center being left alone with children when she was not eligible to be alone with children. An investigation was conducted on March 23, 2022. This complaint is substantiated for improper staff-to-child ratio. The Administrator Jennifer Matlack confirmed that a classroom was out of proper staff-to-child ratio on March 17, 2022, because one staff left the classroom to walk a child to the front door for dismissal. To correct the ratio issues Staff #1 texted Staff #3 and was instructed to allow parents to enter the building to pick up their children. Based on the administrator s statements as well as all documentation provided to LS Brady regarding this complaint, all current staff were eligible to be alone with children.

The Office of Child Care Licensing received a complaint on November 10, 2021, regarding the center possibly not having enough toilets to meet the 1:10 ratio for children 24 months through preschool-age, walls not being cleanable and in good repair, all areas of the center not being in a clean and safe condition due to mildew, and a possible infestation of mice. An investigation was conducted on November 15, 2021. This complaint is substantiated because walls were not cleanable and in good repair and staff and children reported seeing several mice starting on October 15, 2021. An additional concern is a toilet was removed in October 2021 from the bathroom in Room #1015 and has not been replaced to date. There is no evidence to substantiate the allegations regarding the center not having enough toilets to meet the 1:10 ratio for older toddlers through older preschool-age children or the presence of mildew in the center. There was a sufficient number of toilets available for use by children to meet the 1:10 ratio requirement. Neither LS Brady nor LS Flowers observed mildew during the facility visit on November 15, 2021.

Ensure that all cots are labeled in Room 1016.
Replace either the lid or the whole trash can in Room 2003 -- the can must always be covered and the lid flaps were broken.
Ensure medication stored is not expired. Child #2 needs medication that is not expired. Send a picture to OCCL when complete.

Ensure children's attendance records are kept that include exact arrival and departure times. Child #17 was not signed in to room 2014. This was corrected at the visit when Licensing Specialist Elizabeth Brady reviewed attendance records, and notified staff to mark Child #17 as present.
