Replace soap, paper towels in bathrooms. Clean ventilation fan in bathroom by the kitchen, very dirty. Send a picture to OCCL.
The following indoor space items need to be corrected: - The intake fans in all bathrooms and in the two-year-old room need to be cleaned. Send pictures. - Clean the wall in the two's room where there was a water leak and send a picture. - The wall around the toilet in the hallway boys bathroom needs repair from water damage. Send a picture.
The ventilation fan in the hallway girls bathroom is not working. Send a copy of documentation that it is at least scheduled to be repaired. Clean around the faucets/handles in both hallway bathrooms.
The diaper mat in the two's room has holes and needs to be replaced. This was corrected at the visit.
Child #1 and Child #3 need updated health appraisals and immunization records. Child #2 will need blood lead screening.
Fix/replace light bulbs in bathrooms, and classrooms (preschool and toddler). Repair the floor in front of the pantry (water). Ensure hot water is working at all sinks kitchen, bathrooms and classrooms. Cover the trash can in the kitchen/lunchroom for garbage. Send copies of all work orders and pictures of completed repairs.
