OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On December 5, 2025, the Office of Child Care Licensing (OCCL) received a complaint alleging being over-capacity, noncompliance with transportation requirements, operating without electricity or heat, improper care of an infant, inadequate meals being served, and a minor child being responsible for child care duties. An investigation was conducted on December 9, 2025. The complaint was substantiated for violations related to transportation requirements. Ms. Dollard reported she began providing transportation services after the enrollment of Child #2; however, during the annual compliance review conducted on June 11, 2025, she stated transportation was not provided and did not notify Licensing Specialist (LS) #1 of the change following Child #2 s enrollment. Ms. Dollard stated she transported no more than seven children and transported four to five children to a local licensed child care facility, where the children are not enrolled, during the week of November 24, 2025. The vehicle was unavailable for inspection at the time of this visit. The complaint was also substantiated for operation without electricity or heat. Ms. Dollard reported her heat service was discontinued on November 27, 2025, and she was unable to provide care in her home. OCCL was not notified of the utility disruption. Ms. Dollard initially stated she told families to find alternative arrangements when the utilities were shut off, then later stated she transported four to five children to a local licensed child care facility for one day during that week while awaiting restoration of utility services and cleaning of the home, rather than closing operations until services resumed. This complaint is substantiated for improper care of an infant based on observation and review of documentation. Child #2 was observed asleep in a bouncy seat and was not immediately transferred to a crib. Child #2 had only a bassinet available for sleep at the home. An infant feeding schedule and daily log for Child #2 were not available during the visit. Ms. Dollard stated diaper checks occur every two hours; however, regulations require diaper checks to occur hourly, with changes every two hours or as needed. This complaint is substantiated for a minor child, also a household member, being responsible for performing child care duties based on observation and an interview with the child. Child #1 was observed giving directions to children and being asked to complete duties during this visit. There was no evidence to substantiate the allegation of over-capacity care, as Ms. Dollard reported seven children were enrolled, and six children were present during this visit. There is no evidence to substantiate this complaint for inadequate meals being served. However, the meal service described by Ms. Dollard did not align with the posted menu for either breakfast or lunch, and no menu changes were noted. An additional concern noted at this visit was attendance records were neither taken nor available for any dates requested, and Ms. Dollard was unsure which date children were transported to another location and unsure of the exact number of children that were transported. An additional concern was Ms. Dollard not being truthful when providing information due to changing the information when interviewed regarding how care was handled when utilities were shut off in the home.
