The Office of Child Care Licensing (OCCL) received a complaint on December 5, 2025, and two additional complaints on December 11, 2025, regarding concerns of lack of supervision and failure to properly document an incident resulting in injury to the face. An investigation was conducted from December 11, 2025, through December 15, 2025, during which Licensing Specialist #1 engaged in phone and email communication with Reporter #1, interviewed Administrator #1 and Staff #1, obtained documentation, and reviewed video footage of the incident. The complaint is substantiated for failure to complete an incident report in a timely manner, as the incident report was not completed until two days after the incident occurred. There is no evidence to substantiate the allegation of lack of supervision, as Staff #1 was able to recall the location, timing, and sequence of events related to the incident.
