On February 20, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding use of improper behavior supports. This incident was self-reported to OCCL by the administrator on January 13, 2026. An investigation began on February 20, 2026, and concluded on February 27, 2026. Based on interviews conducted and documents reviewed, while there is no evidence to substantiate this complaint for use of improper behavior supports regarding the incident reported to have occurred on January 9, 2026. However, this complaint is substantiated for use of improper behavior supports on previous occasions based on interviews with multiple staff members who stated they have witnessed other incidents when Staff #1 interacted inappropriately with a child in care. Five staff members stated they have seen Staff #1 appearing to be frustrated with younger children and two staff members observed Staff #1 grab or pull children aggressively. Staff #1 completed two professional development trainings, The Compassionate Coach: Building Bridges to Support Educators with Challenging Behaviors and Coaching for Change: Working through Resistance, provided by Early Childhood Investigations Webinars.

Remove tree stumps from playground that pose tripping hazards. Mulch is needed in bare area used for child play and remove tripping hazards.
Ensure infant feeding schedules are updated monthly for all infants. Forward for Child #3 and #5.
Ensure monthly fire prevention inspections are completed. Forward for August 2022. July 2022 was not conducted.
Replace torn nap mats that are no longer non-absorbent.
