The Office of Child Care Licensing (OCCL) received a complaint on February 13, 2026, regarding concerns of unqualified staff alone with children, failing to notify parents of an injury, staff using intoxicating substances while working with children, and failing to use appropriate behavior supports. An investigation was conducted on February 19, 2026. There is no evidence to substantiate this complaint for allegations of unqualified staff alone with children based on classroom observations and a review of attendance records and staff file documentation. There is no evidence to substantiate this complaint for failing to notify parents of an injury based on staff interviews conducted. Administrator #1 and three staff members interviewed stated families are notified of any incidents or accidents. No staff were aware of an injury occurring to Child #1 while in care. There is no evidence to substantiate this complaint for staff using intoxicating substances while working with children based on interviews conducted. Administrator #1 and eight staff members interviewed all stated they have never observed any staff members come into the facility under the influence of intoxicating substances and have never done so themselves. No staff members present at the facility appeared to be intoxicated. There is no evidence to substantiate this complaint for failing to use appropriate behavior supports based on observations and interviews conducted. Administrator #1 and eight staff members all denied witnessing any staff not using positive behavioral supports and stated they only use positive behavior management.

The Office of Child Care Licensing (OCCL) received a complaint on February 20, 2026, regarding concerns of lack of supervision due to sleeping in the classroom, using inappropriate behavior supports, failing to understand the needs of children, interactions with infants, staff under the influence of drugs, and unqualified staff. An investigation was conducted on February 26, 2026. Based on interviews conducted and observations at the visit there is no evidence to substantiate this complaint for lack of supervision due to staff sleeping in the classroom, failing to use appropriate behavior supports, failing to understand the needs of children, inappropriate interactions with infants, or staff under the influence of drugs. All staff interviewed denied participating in or observing any of these allegations. Based on classroom observations and attendance records reviewed, there is no evidence to substantiate this complaint for allegations of unqualified staff alone with children. Classrooms observed at the visit all had staff members qualified through DEEDS Early Learning. All attendance records reviewed reflected staff members were qualified through DEEDS Early Learning.

The Office of Child Care Licensing (OCCL) received a complaint on November 13, 2025, regarding concerns of not following positive behavior supports. An investigation was conducted on November 19, 2025. This complaint is substantiated for not using positive behavior management strategies. LS #1 interviewed Administrator #1 and 11 staff members during this investigation; 10 of those staff members denied witnessing any staff not using positive behavioral supports and stated they only use positive behavior management techniques themselves. One staff member stated they have observed questionable interactions but did not elaborate further. However, a walkthrough of the facility and observation of classrooms showed a staff member using prohibited acts of behavior management. This staff member was observed yelling in a child's face and roughly placing the child back into a chair for a time out in the two-year-old classroom.

The Office of Child Care Licensing (OCCL) received a complaint on October 17, 2025, and on October 20, 2025, from the same reporter, regarding concerns of not following and utilizing positive behavior supports due to sending a child home and not documenting behavioral incidents or developing a plan with the families. An investigation was conducted on October 29, 2025. There is no evidence to substantiate this complaint for not following and utilizing positive behavior supports based on staff interviews conducted. Administrator #1 stated on multiple occasions they have either scheduled meetings to develop a plan for Child #1 or invited Parent #1 to view Child #1 on the livestream video footage, and Parent #1 has not attended. Administrator #1 and three staff members stated they have only observed positive behavior management used within the facility. Administrator #1 and three staff members interviewed stated all challenging behaviors are documented, and copies of this documentation were viewed at this visit.

The Office of Child Care Licensing (OCCL) received a complaint on September 17, 2025, regarding concerns of not using respectful communication with families and not following termination policies. An investigation was conducted on September 23, 2025. There is no evidence to substantiate this complaint for not using respectful communication with families and not following termination policies based on staff interviews and review of the center s parent handbook. Administrator #1 and Staff #1 stated respectful communication was used with the family, and all of the center s parent handbook policies were followed regarding termination of care for a child.

An investigation was conducted on July 22, 2024. Staff members stated Child #1 was not observed being hurt and stated Child #1 never told them about an injury. Staff members stated a report was written after Parent #1 expressed concerns and took Child #1 to the doctor. The Office of Child Care Licensing (OCCL) received a complaint on July 5, 2024, regarding concerns about not using respectful communication, including sharing confidential information, staff using loud/threatening tones in the facility, not following staff policies, and not following illness policies. An investigation was conducted on July 10, 2024. This complaint is substantiated for staff using loud/threatening tones in the facility based on interviews conducted with staff members. Staff interviewed stated Staff #1 did use a loud/threatening tone towards an administrator. While responses were mixed, three staff members also stated the administration team has been seen being unprofessional by using inappropriate tones and belittling in front of others. There is no evidence to substantiate this complaint for concerns about sharing confidential information based on interviews conducted. All staff members interviewed stated no confidential information is shared throughout the facility that they are aware of. There is no evidence to substantiate this complaint for not following staff policies based on interviews conducted and observations of facility policies during this visit. Staff interviewed stated staff must work the day before and after a holiday to get paid, which matches the staff handbook. There is no evidence to substantiate this complaint for not following illness policy based on interviews conducted. All staff members stated families are always notified of a contagious illness via the applications used by the facility or by a sign on the classroom door, and noted children are picked up as soon as they show signs of illness. Staff interviewed stated children do not return until symptom-free or with a doctor s note.

On May 16, 2024, a complaint was called into the Child Abuse and Neglect Report line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 16, 2024, and accepted it for investigation regarding concerns of improper discipline. An investigation was conducted on May 16, 2024. There is no evidence to substantiate this complaint for improper discipline based on interviews conducted. Staff members interviewed stated they had never witnessed any improper discipline and stated they had not witnessed Staff #1 pin a child against a wall with the crib. An additional concern noted was the facility was notified of suspected abuse of a child in care and did not make a report to the Child Abuse and Neglect Report Line. An additional concern noted at the visit was a staff member was observed on a cell phone while caring for children.

The Office of Child Care Licensing (OCCL) received a complaint on January 22, 2024, regarding concerns of lack of supervision, staff performing other duties such as using personal cellphones while caring for children, and not providing truthful information to a parent or OCCL. An investigation was conducted on January 30, 2024. This complaint is substantiated for lack of supervision based on attendance records reviewed from January 18, 2024. Attendance records reviewed showed two classrooms over staff-to-child ratio and one classroom over maximum group size on this date. This complaint is substantiated for Staff #1 using her cell phone in the classroom with children present. This complaint is substantiated for not providing truthful information based on documentation collected at the visit and interviews with staff members because Staff #1 first indicated to Parent #1 that nothing was around Child #1 to cause an injury but later stated Child #1 had a shaker toy. During this investigation, LS Daffern requested the timecard for Staff #2 and was provided with contradicting information and times from staff. An additional concern noted at this visit was incomplete attendance records for children and staff members, as there were several records with the children and staff not being signed in and out or documenting transitions.

An investigation was conducted on October 3, 2023. This complaint is substantiated for administration not being available to help based on observation and interviews conducted with staff members. There is no evidence to substantiate this complaint staff members having other duties or responsibilities based on interviews conducted. There is no evidence to substantiate this complaint for improper staff-to-child ratios based on interviews conducted and observations during this visit. All classrooms were within proper staff-to-child ratio during the visit. There is no evidence to substantiate this complaint for not notifying families of exposure to contagious illness based on interviews conducted. All but one staff member interviewed noted families are always notified of a contagious illness via the applications used by the facility or by a sign on the classroom door, and Administrator Debra Romish stated no children have been sent home sick recently.

An investigation was conducted on August 11, 2023. This complaint is substantiated for failing to release a child to a parent and providing untruthful information based on interviews conducted. While the staff members interviewed stated they were verifying the information on the custody agreement for Child #1, a copy of the agreement clearly states Parent #2 had custody of Child #1 on the date of this incident.

An unannounced visit was conducted on July 5, 2023, to investigate this complaint. This complaint is substantiated for improper staff-to-child ratios because multiple classrooms were out of ratio based on a review of attendance records. Additional concerns noted were maximum group size was exceeded in three classrooms, an unqualified staff was alone with children, toddlers were not cared for in an area physically separate from the older children, and staff and child attendance records were incomplete.

The Office of Child Care Licensing (OCCL) received a complaint on June 30, 2023, regarding concerns about a child not receiving their medication. An investigation was conducted on July 5, 2023. There is no evidence to substantiate this complaint for a medication error because the facility notified Parent #1 there was no medication left. Parent #1 continued to drop off Child #1 without the medication for three days.

The Office of Child Care Licensing (OCCL) received multiple complaints between June 1, 2023, and June 9, 2023, regarding improper staff-to-child ratios, maximum group size requirements not being followed, and children being cared for in an unlicensed area. An investigation was conducted on June 6, 2023. Based on the documentation provided, the complaints are substantiated. Attendance records showed proper staff-to-child ratios were not followed in the Preschool I classroom on May 24, 2023, and in the two-year-old classroom on June 2, 2023. The records also verified maximum group size was exceeded in the Infant A classroom on May 30, 2023, and in the Infant B classroom on June 2, 2023. Two staff interviewed stated some classrooms are over ratio in the morning depending on staff s schedules. Attendance records dated May 31, 2023, also showed two children were cared for at the front desk, which is an area not approved for child care. Additional concerns noted were arrival and departure times for staff and children were not accurately documented.

Based on an interview and documentation received, this complaint is substantiated for not clearly communicating with enrolled families. The facility utilized a means of communication not listed within the parent handbook or enrollment agreement. The messages sent to the families did not align with the information provided to families verbally on June 19, 2023. There is no evidence to substantiate for improper transportation based on an interview conducted. Ms. Romish stated she is unaware of an incident with any child being returned to their elementary school by their school bus. Ms. Romish noted an incident where a driver attempted to drop a child off on the wrong side of the building, but Ms. Romish stated brought this child into the facility.
