On September 4, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding the center s food service policy for addressing food allergies, serving food to a child with known allergies, and not immediately providing medical attention, then notifying parent when child has an allergic reaction. An investigation was conducted on September 9, 2024. Based on observations, interviews with staff, and documentation, this complaint is substantiated for not following the center s food service policy addressing food allergies. Although Staff #1, Staff #2, and Administrator #2 noticed Child #1 s menu was not highlighted indicating it was permissible to feed them chicken salad, they still served Child #1 the chicken salad sandwich. Based on observations, interviews with staff, and documentation reviewed, this complaint is substantiated for serving food to a child with known allergies. Although Staff #1 and Staff #2 noticed Child #1 was not permitted to have the chicken salad sandwich, they prepared and served the sandwich to them. Based on interviews with the staff there is no evidence to substantiate for not immediately providing medical attention before notifying the parent. Staff #1 and #2 both stated Child #1 had no reaction after eating the sandwich, or before nap. Staff #1 stated Child #1 only vomited and started coughing towards the end of naptime, a few hours later. At that time, the action plan was followed, and the parent was notified. An additional concern noted after a review of documentation was that the Medication Administration Record (MAR) form for Child #1 had not been documented as having been reviewed and updated yearly as required.

The Office of Child Care Licensing received a complaint on October 18, 2022, regarding the center not allowing parents access to the building, and staff not allowing qualified professionals to complete an observation or assessment of a child while at the center. An investigation was conducted on October 19, 2022. This complaint is substantiated for staff not allowing parents/guardians to enter the facility because staff stated parents were not permitted to come into their child s classrooms and parents were observed waiting outside of the center to pick up their children at the end of the day, and stated they were unaware they were allowed to enter the center and go to their child s classroom. This complaint is substantiated for staff not allowing qualified professionals to complete an observation or assessment of a child while at the center because Ms. Mock stated she was unaware a center staff member did not need to be present when therapists were working with individual children at the parents request when talking to a parent who was trying to schedule a therapist to assess their child while in care at the center.
