Ensure staff files are completed. Staff #1 needs orientation, job description, references, release of employment and service letters.
Ensure child files are completed. Child # 3, 5, 6, and 9 need blood lead screening. All infants need a curretn feeding statement. Child #10 needs a health appraisal, immunizations, action plan, MAR and an EpiPen in the original box.
Ensure outdoor are fall zones requirements are met. At the bottom of the slide, add mulch and/or mat. Send picture to OCCL.
Ensure monthly fire drills are conducted. Send December and January fire drills when complete.
The Office of Child Care Licensing (OCCL) received a complaint on September 11, 2023, regarding incorrect staff-to-child ratios and infants being kept in confining equipment. An investigation was conducted on September 18, 2023. This complaint is substantiated for incorrect staff-to-child ratios based on the administrator s statements and observations at the time of the visit. There is no evidence to substantiate this complaint for infants being kept in confining equipment for more than thirty minutes, based on observations in the infant classroom and interviews with multiple staff.
Disposition: Substantiated
On January 11, 2022, OCCL received a complaint alleging failure to follow correct staff-to-child ratios. Based on observations made during the investigation visit, the complaint is substantiated for violation of staff-to-child ratios. Two classrooms were observed with children sleeping and no staff member was present in the room. Additionally, toddlers and preschoolers were combined outside the permitted times. During the visit, children under two-years-old were observed participating in screen time.
