On May 12, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegation of staff smoking while transporting children, a person present in the home yelling at children, and the provider getting into a physical and verbal altercation with a parent. An investigation was conducted on May 22, 2025. There is no evidence to substantiate this complaint for staff smoking while transporting children. Provider #1 denied ever smoking around children and denied that Household Member #1 ever smoked around children or while transporting children. There is no evidence to substantiate this complaint for the provider yelling at the children. Provider #1 denied ever yelling at children and denied Person #1 ever yelling at the children. There is no evidence to substantiate this complaint for the provider getting into a physical and verbal altercation with a parent. Although an incident took place, Provider #1 denied touching Parent #1 or yelling at Parent #1. Provider #1 stated they asked Parent #1 to leave and shut the front door. An additional concern was noted that the emergency substitute was providing transportation to children during inclement weather. This is not an approved use of an emergency only substitute.

On October 11, 2023, a complaint was called into the Child Abuse and Neglect Report Line and was not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on October 12, 2023, and accepted it for investigation regarding improper discipline on October 12, 2023. An investigation was conducted on October 20, 2023. There is no evidence to substantiate this complaint. Ms. Rogers denied calling any children names and stated she would never push a child or call them an inappropriate name. Ms. Rogers noted Child #1 was acting aggressively toward her. Ms. Rogers mentioned Parent #1 was aware of the situation and was contacted immediately after Child #1 refused to put on her coat.
