OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On May 15, 2025, an incident of staff failing to use appropriate behavior supports was self-reported to OCCL by the facility. On June 3, 2025, the Office of Child Care Licensing (OCCL) received two complaints regarding the same incident. An investigation was begun on May 15, 2025, and concluded on August 13, 2025, after clearance from other agencies was received. This complaint is substantiated for staff failing to use appropriate behavior supports. A self-report from Administrator #1 and a review of camera footage showed Child #1 was restrained and handled roughly by Staff #1. As a result, Staff #1 s employment was terminated by the facility.
