On March 7, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of lack of supervision, failure to report a child injury requiring medical attention to OCCL within one business day, and failure to keep all indoor spaces hazard-free. Based on interviews, this complaint is substantiated for failure to report a child injury requiring medical attention to OCCL within one business day. The center did not notify OCCL of an injury requiring medical care within one business day of being notified medical care was needed. Based on interviews conducted and observation during the visit, there is no evidence to substantiate this complaint for failure to keep all indoor spaces hazard-free. All staff interviewed stated the door was not broken. Based on interviews conducted, there is no evidence to substantiate this complaint for lack of supervision. The staff member was present and supervising children but was not close enough to intervene before the injury occurred.
