The Office of Child Care Licensing (OCCL) received a complaint on March 17, 2026, regarding concerns of a child ingesting an unknown medication due to a medication not being stored securely and out of children's reach. An investigation was conducted on March 18, 2026. Administrator #1 and two staff members stated Child #1 did put an unknown pill in their mouth, but the pill was removed immediately, and all policies and procedures were followed. It was not able to be determined how the medication came to be in the classroom. All staff interviewed denied having any such medication at the center. While a child did access some type of medication found on the floor of the classroom, there was no evidence of incorrect storage of classroom medications or lack of supervision; therefore there is no evidence to substantiate this complaint.

An investigation was conducted on January 22, 2026. Administrator #1 and two staff members all stated children are using safety restraints during transportation, and the document reviewed showed name-to-face and seat belt checks were completed.

Administrator #1 stated Child #1's bottles were not labeled, which led to Staff #3 not feeding Child #1 throughout the morning. Administrator #1 and Staff #1 stated Staff #2 was lifting children by one arm to move them to a different area in the classroom, and Staff #2's coaching form stated Staff #2 demonstrated how they lifted the children, which was lifting children by one arm. The Office of Child Care Licensing (OCCL) received a complaint on October 23, 2025, regarding concerns of not following positive behavior support. An investigation was conducted on December 5, 2025. There is no evidence to substantiate this complaint for concerns of not following positive behavior support, based on staff interviews conducted. Administrator #1 and eight staff members were interviewed separately and stated they have only observed appropriate positive behavior management within the facility and stated they only use appropriate behavior management support. A concern noted during the visit and review of attendance records was that staff members worked alone with children who were unqualified through DEEDS Early Learning on multiple occasions. In the walkthrough of the facility during the investigation, four classrooms, including two infant rooms and two toddler rooms, had unqualified staff working alone with children. Attendance records reviewed for the week of November 17, 2025, through November 21, 2025, reflected an Early Childhood Intern and staff members without a DEEDS Early Learning qualifications were alone with infants in both Infant classrooms for the entire week of attendance viewed. Staff members with no DEEDS Early Learning qualification were signed in alone with one-year-old children for the entire week in all three toddler classrooms. There were four instances throughout the week of November 17, 2025, where staff without qualifications were alone in two-year-old and preschool-age classrooms as well. An additional concern noted was a staff member signed in to work with children alone on multiple occasions without having a comprehensive background check completed. Another concern noted is inaccurate attendance records as there were several instances of staff not signing out of their classrooms from November 17, 2025, through November 21, 2025, and instances where no coverage of classrooms was shown.

On June 5, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns regarding meeting sanitation requirements due to excessive illness within the center. An investigation was conducted on June 9, 2025. There is no evidence to substantiate this complaint based on interviews conducted. Administrator #1 and eight staff members interviewed stated classrooms and toys are cleaned and sanitized daily, toys are sanitized when mouthed, and classrooms and toys are continuously sanitized when an illness is present in the facility. No cleanliness or sanitation concerns were noted during the walkthrough of the facility, and proper cleaning solutions were observed in each classroom.

There is no evidence to substantiate this complaint for improper discipline based on interviews conducted. There is no evidence to substantiate this complaint based on interviews conducted. Administrator #1 and four staff members interviewed stated they have not witnessed any improper discipline within the facility. One staff member stated Staff #1 nudged a child on the head. All staff members interviewed stated they have not used improper discipline.

A complaint was received on March 14, 2022, regarding an unreported and undocumented serious injury to a child in care that occurred on March 20, 2020. An investigation was conducted on March 23, 2022. There is no evidence to substantiate this complaint for unreported and undocumented serious injury because the only incident that the center staff could note when the child was crying was when he was on the playground in the afternoon shortly before he was picked up by his parent, but he was quickly soothed and did not continue to show signs of discomfort or injury at that time. Once Ms. Orsini was informed that the child was being taken to the hospital, Ms. Orsini stated she contacted OCCL to report the incident. The center was closed on what would have been the next business day, March 23, 2020, until June 14, 2020, due to the COVID-19 pandemic. The incident report was forwarded to both the parent and OCCL on Tuesday, March 24, 2020.
