An unplugged crockpot was observed full of leftover food on the kitchen counter. An unclean spoon was observed next to the crockpot. Ensure food is clean stored properly. A frozen waffle was observed on the counter, two waffles were observed in the toaster, and the remaining waffles in the packaging were observed unfrozen on the kitchen counter. Ensure the freezer is at 0 degrees F or colder. The thermometer for the freezer read 1 degree F and 2 degrees F.
On July 9, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, not following the policies in the handbook, and not being respectful. An investigation was conducted on July 23, 2025. An investigation was conducted on June 5, 2024. The complaint is substantiated for the suspension/expulsion policy because there was not a suspension/expulsion policy in the parent handbook the provider provided during the visit. The allegation of not following accident/injury procedures is substantiated because the accident/injury handwritten report was missing the child s name and the first aid or medical care provided as required in the regulations and was not available in the child s file. There is no evidence to substantiate the allegation of lack of supervision because the provider stated the incidents happened when she was changing a diaper, preparing a meal, or assisting another child in the bathroom.

When a child is placed in time-out, they may not be restained. Upon arrival, LS Sommer observed one child in a feeding chair with a connected T-Strap and was quickly removed by Ms. Dismuke. During the visit, Ms. Dismuke placed the same child back in the feeding chair, verbally informed him it was for time-out, and clipped the T-Strap.
