The Office of Child Care Licensing received a complaint on March 14, 2025, regarding lack of supervision, hazardous materials in the children s reach, and vaping. This complaint is substantiated for lack of supervision and hazardous materials in the children s reach because Staff #1 admitted the pen must have fallen from their pocket and was picked up by Child #1 unnoticed by the staff. There is no evidence to substantiate the allegation of vaping because staff did not indicate they were vaping in the facility and the staff handbook prohibits vaping.

The Office of Child Care Licensing (OCCL) received a complaint on March 30, 2023, regarding improper discipline, staff communication, and altering attendance records. An investigation was conducted on April 4, 2023. There is no evidence to substantiate the allegations of improper discipline, disrespectful communication, or altering attendance records because the staff interviewed denied the allegations. A concern noted during the interviews was a staff member who was recently rehired was present before receiving an updated eligibility determination.

Disposition: No Evidence to Substantiate with Concern

OCCL received a complaint on December 28, 2021, improper staff-to-child ratio and staff wearing their mask improperly. A telephone investigation and document review were conducted on December 29, 2021. There is no evidence to substantiate this complaint because review of the classroom attendance showed the rooms were in proper staff-to-child ratio and no one in the receiving classroom admitted to wearing their mask improperly.
