On August 2, 2021, a complaint was accepted for investigation by OCCL regarding a staff member vaping marijuana on the job, and a staff member not having a health appraisal. LS Donovan investigated the complaint on August 9, 2021. There is no evidence to substantiate that Staff #1 was using drugs on the job. However, this complaint is substantiated because both Staff #1 and Staff #2 did not have health appraisals. Additional concerns were staff members files were missing additional documentation including references, service letters, a verification of fingerprinting, eligibility letters from the Criminal History Unit, an employee declaration, and a drug and alcohol prohibition acknowledgement.
