The Office of Child Care Licensing (OCCL) received a complaint on October 13, 2025, regarding staff not receiving the staff handbook, staff not completing the orientation, outdoor area, indoor space, staff and child attendance, not keeping medications out of the children s reach, not keeping chemicals out of the children s reach, having chemicals in the labeled soap/water bottles instead of soap and water, not having paper towels for handwashing, not following meal pattern requirements, and allowing children to walk unsupervised to the bus without parent permission. An investigation was conducted on October 23, 2025. This complaint is substantiated because water bottles containing chemicals were labeled as soap and water, wires on the small playground were exposed, and the fence on the large playground did not have functioning self-closing and self-latching mechanisms. There is no evidence to substantiate staff not being oriented, staff not receiving a handbook, attendance records not being used, medications being stored in children s reach, unlocked cabinets with chemicals in children s reach, a child spraying chemicals in their face, inadequate portions sizes served at meals, and school-age children being unsupervised at the bus stop based on interviews and observations.

OCCL received a complaint on November 12, 2021, regarding not following policies in the parent handbook. An investigation was conducted on November 15, 2021. This complaint is substantiated because Ms. Edwards did not follow the termination policy written in her parent handbook. Ms. Edwards stated she terminated care due to feeling her business was being attacked, which is not in her handbook for reasons for termination.

OCCL received a complaint on September 22, 2021, regarding improper discipline. An investigation was conducted on September 23, 2021. The complaint is substantiated for improper discipline because according to Ms. Edwards and the discipline record provided, Staff #2 sat a child down roughly on a bench. Ms. Edwards observed this on video footage that is no longer available. Other concerns noted were improper staff-to-child ratio in five classrooms and on the playground; lack of supervision because four children were left in a classroom unattended; four rooms were being covered by two staff, and a child left the playground; and incorrectly documenting staff and children s attendance in the one and two-year-old classrooms.
