Director will ensure that all personnel files are complete and accurate with all required documentation needed. Director will also create an updated Personnel Record (LIC 500) form. These tasks are to be completed no later than the due date of 02/27/2026. Based on record review, LPA Uribe found that several required documents were either incomplete or missing from personnel files and an incomplete Personnel Record (LIC 500) which poses a potential risk to the heatlh, safety, and personal rights to children in care. LPA Uribe will return to the facility shortly after the 02/27/2026
