The facility will ensure that all children remain under direct visual supervision at all times. The facility will install safety gates on all preschool main door entrances. Additionally, to prevent unauthorized or unnoticed exits, the facility will Install a doorbell-style alarm on the main entrance door that alerts staff whenever the door is opened. Train staff on responding immediately when the alarm sounds. Based on interviews, document review, and camera footage, LPAs determined that on 05/01/2026, C1 ran out of the facility onto the sidewalk without proper supervision, which poses/posed an immediate health, safety or personal rights risk to persons in care. Licensee agrees to provide a written statement (LIC 855 Declaration Form) on intention to keep children safe at all times. Declaration to be submitted to LPA via email by POC due date.
