The infant/toddler intake information for Child 3 was incomplete upon review. Written information from Child 3's parent/guardian to individualize the program of care was lacking on two of the five page form.
Staff preparing lunch at the time of licensing visit lacked a proper hair restraint, such as a hair net or cap.
During licensing visit, two children on different occurrences had their movements restricted when they were confined. A child was observed in a packnplay multiple times during visit, child was not resting or provided with activities. A different child was observed in a highchair upon licensing arrival and at times throughout visit, child was not eating while in the chair, nor was child given activities.
A medication form was observed on the premise without an end date.

Enrollment information was incomplete for Child 3 upon licensing review. The file lacked information pertaining to childs first date of attendance and parental contact information while child is in care.
An infant in care at the time of licensing visit was not being cared for in his assigned room. Child was observed being cared for in the "Red Room" throughout licensing visit. The "Red Room" is not licensed for children under 2 years old.
The health history and emergency care plan was incomplete for Child 1, upon licenisng review. The form noted a medical condition but lacked information pertaining to the emergency care plan and steps to take.
The file for Staff A lacked a certificate from the Registry documenting staff is qualified for the position held. Staffs registry certificate did not document staff was qualified for a position held (teacher).
A fire extinguisher on the premises lacked an inspection within the past year. The last documented inspection was January 2025.
Center medical log book lacked documentation of a review within the past six months to ensure all possible preventive measures have been taken. The book started April 2025, no reviews were documented.
