The implemented tracking procedure failed when a child was left outside on the playground and not accounted for with his class. Child was located with a different group for almost 30 minutes and was still listed on the initial classroom tracking form.

Staff has been employed longer than 6 months. A Warning Letter was issued February 18, 2026.

A child was not within sight and sound supervision of a child care worker to guide behaviors, ensure safety, and prevent harm when the child was left outside on the play ground, alone, for three to four minutes. Staff became aware of child's location when child knocked on the door to a different classroom from the outside. A Warning Letter was issued February 18, 2026.
Minimum meal requirements were not met on several occurrences when children were not provided a beverage at meals and snacks. Daily meal documentation on the center app noted children being served foods without beverages, photos showed the same.
Children in the infant room were observed sitting in highchairs without infant safety straps. The straps were located and reinstalled in the highchairs during visit.
The indoor temperature was not maintained at a minimum of 67 degrees. The infant classroom was noted with a digital thermometer as low as 55 degrees near the cribs and where infants were playing on the floor. The Pre-K classroom was noted at 61 degrees.
Staff B lacked qualifications of an assistant child care teacher, staff has been employed longer than 6 months. The same staff member was noted in a June 2025 violation; the correction plan noted a completion date of July 2025 for the course.
Center name-to-face tracking sheets noted one staff member with 13 children (at a prorata calculation of 1.232), until a second teacher arrived, on the day of licensing visit. On January 28, 2026, a second staff member signed into the preschool classroom at 8:07am with 15 children present in the room (the prorata ways 1.422). Earlier, one staff member was present in the room with 12 children, ranging in age from 2yo to 5yo with a prorata of 1.12.
Dry foods, including flour and pasta, were not stored in bags with zip-type closures of food-grade containers with tight-fitting covers, and labeled.
A toilet room in the two-year-old room was not maintained in a sanitary condition. Toilet was observed backed-up with a large amount of toilet paper and refuse.
Center failed to ensure that any action, by commission or omission, did not adversely affect the health, safety and welfare of children under the care of the licensee. Center staff stated a child in care was served milk which expired over a week prior.
The premise was not free from potential sources of harm at the time of licensing visit. Plastic gloves were accessible to children in the PreK classroom on the children's sink area.
