Center was unable to provide manufacture information for a equipment utilized in the outdoor playspace that has caused injury for two year olds. Center did find that the equipment is intended for 5-12 year old children and did block off the equipment when realizing this.
The outdoor play space lacked at least nine inches of energy absorbing materials at the time of the licensing visit. A child was injured in the outdoor play space and self reported.

Prohibited actions that physically, psychologically or emotionally injured a child were utilized in the childcare setting by an employee when the employee threw a child's toy, (a hard truck) at the child causing pain to his ear and causing the child to cry.
Staff were not able to provide sight and sound supervision to a child when a family member arrived to pick up a child and the child was unable to be located in the classroom. Child (3 years old) was outside of classroom for "reported" as roughly one minute.
Teachers did not report unknown whereabouts to DCF within 24 hours after incident. Parent reported to Director and director notified DCF outside of the required timeframe.
Child tracking procedure implemented failed when a three year old child walked out assigned classroom and was not present, location unknown to caregivers until the parent came to pick up child and child was not within the room being tracked.
Child 1 and child 4 had incomplete authorized persons and emergency contact information available for review. Child 2 had missing emergency contact information available for review.

(Mandated Reporting - Child Abuse & Neglect ) on 10/22/2024 : Center failed to immediately contact the county department of social services when learning of an abuse allegation involving an employee and a child in care. No Staff that were aware of the allegation reported the incident and were instructed not to as the Regional Office for Kindercare instructed them that they would handle the incident and reporting. The County Department of Social Services was not notified until Licensing Specialist instructed the Director to do so as a Mandated Reporter. This violation was previously included in an Order dated January 25, 2024.
