An alternative arrival form was not completed for a child that arrives from a 4K offsite school to the daycare center.
There were 4 children in the 4 K room that were out of the sight of the two classroom teachers while using the bathroom and washing up prior to lunch time. (They were within sound as it is connected to the classroom.) It should be noted that the director noticed this and stepped in for supervision and assistance.

Staff did not follow center policy that name to face tracking will be completed when moving from one location to another. After watching video for an incident that occurred on 2/11/25 the toddler classroom leaving the indoor gym did not perform name to face tracking when leaving the gym. The Pink Room did not perform name to face tracking when entering or leaving the indoor gym. The same classroom did not do name to face tracking when entering or leaving a different gym to play in. The same classroom did not do a name to face tracking when they returned to their classroom.

During review of video camera footage two childcare workers were observed looking down on their phones, not providing sight supervision to the children.
