According to staff statements, 3 children in care in the Yellow Room eloped from the assigned classroom staff and travelled down the hallway and into a bathroom. A staff member found the children playing in the toilet water, potentially ingesting it. It was observed during the complaint visit that the toilets contained toilet bowl cleaning product, resulting in children potentially ingesting poisonous chemicals at the time of the incident.
Rule requires all licensees to report to the department within 24 hours of any incident where a child's whereabouts are unknown to the child care provider. The licensee was aware of an incident where children's whereabouts were unknown and did not report to the department.
(Supervision Of Children ) on 2/24/2026 : According to staff statements and video footage, 3 children were not within sight and sound supervision to prevent harm and ensure safety for approximately 10 minutes on 2/13/2026. According to staff statements, 3 children in the Yellow Room had left their assigned group, and travelled down the hallway to the bathroom, where they were found playing in the toilet. One of those 3 children escaped a second time and was found by himself in the center kitchen. It was observed on the center's video footage that the 3rd child was returned to the classroom several minutes after the first 2 children. $200

(Child Tracking Procedure ) on 2/24/2026 : The center's child tracking procedure was not followed when 3 children in care in the Yellow Room left their assigned group and travelled down the hallway to the bathroom. The children were noticed playing in the toilet in the hall bathroom by a staff member from a classroom across the hallway, and an additional staff member in the center collected and returned the children to the Yellow Room. After the 3 children were returned to their classroom, one of the 3 children escaped again and was found in the center kitchen, according to staff statements. $200

Upon a review of attendance records on 2/13/2026, it was discovered that there were 10 children under age one in care in the Green Room for approx. 55 minutes. The maximum group size for children under the age of 18 months is 8.
It was self-reported by the center that on 11/10/2025, a 2 year old child was left alone unsupervised in the enclosed outdoor play area for approximately 10 minutes when his assigned caregivers and classmates went inside without him.

On 11/10/2025, center staff did not adhere to the center's child tracking procedure to ensure that the number, names, and whereabouts of children in care are known to assigned child care workers at all times when a class came inside from the outdoor play area and did not successfully track every child's whereabouts, resulting in one child being left alone outside for approximately 10 minutes between 4:06pm and 4:16pm.

The licensee failed to report to the Department within 24 hours of law enforcement contact at the center. It was discovered during a complaint investigation that the police came to the center on 10/14/2025, regarding an incident that occurred at the center earlier that day.
The attendance was not current and accurate during the first hour of operation on the day of the visit when two children were signed into the Red Room but were actually in care in the Green Room, and four children were signed into the Pink Room but were actually in care in the Green Room, according to staff statements.
It was observed during the visit that only one staff in the Green Room was supervising seven awake children and one sleeping child under the age of 1 year old for a period of time while the other assigned caregiver was in the bathroom.
It was reported by multiple staff that on 10/14/2025 the Owner/Administrator made remarks that disclosed personal information about a child's relatives in a manner that was loud enough for staff, children, and other families to hear that were not privy to that information.

Upon review of attendance records from 11/10/2025, the center did not have accurate documentation of days & hours worked and in which specific classrooms for staff who are being counted in staff-to-child ratio. Attendance records indicated that staff were not signed into the room in which they were counted in staff-to-child ratio for a period of approx. 30 minutes when multiple groups of children were outside, & again between 4:06pm and 5:30pm when staff were not signed into the room in which they were providing care and supervision to children.
It was reported by multiple staff that the Owner/Administrator made derogatory remarks about a child's family to the 3 year old child on 10/14/2025, which may have been emotionally or psychologically discomforting for the child.
