An Order Letter with an Imposed Plan of Correction that was issued to the program on March 19th, 2026, was not posted and visible to parents as is required by rule.
The attendance record was not current and accurate on the day of the licensing visit. When the licensing representative arrived, there were 19 total children in care of the center, and only 15 children were signed in. Later during the visit, after 2 children were picked up, there were 17 total children in care of the center, and only 13 were signed in.
There was no physical examination on file for Staff D. Staff D has been employed by the center and providing care and supervision to children for over a year. Rule requires that a physical examination report is completed within 12 months before or within 30 days after beginning work with children in care, indicating the person is free from illness detrimental to children, including tuberculosis, and physically able to work with young children.

There was no documentation of a completed orientation for Staff E on file for review by the licensing representative. Rule requires that the licensee shall maintain documentation of a completed orientation for each adult who works at the center within their first week of work.
A child care teacher who has been providing sole supervision to groups of children did not have documentation of the entry level training requirements for the position that they hold.
There is no documentation of days and hours worked by each staff member, and in what classroom they are counted in staff-to-child ratio, as is required by rule.
There was no record of continuing education on file for Staff D, Staff F, or Staff G available for review by the licensing representative for the year 2025.
Staff D was missing documentation of having maintained a current certificate of completion for infant and child cardiopulmonary resuscitation (CPR) and automated external defibrillator (AED) use from an agency approved by the department. Staff C has a current CPR-AED certificate, but not from a department-approved course provider nor including infant and child CPR training components.
An assistant teacher is required to complete entry level training within 6 months of the hired date. There was no documentation of completion of a non-credit department-approved course or a credit course in early childhood education for Staff D, who has been employed at the center for longer than 6 months. This unqualified staff has been counted in staff-to-child ratio and providing supervision to groups of children.
It was observed by the licensing representative during the visit that a 17 year old staff member was providing sole supervision to a group of napping children. Rule states that no person under 18 years of age shall be left in sole charge of a child.
It was observed during the licensing visit that staff-to-child ratios were not maintained in the center during naptime. The Assistant Teacher in the Infant Room left the premises for a period of time during naptime on the day of the licensing visit, and there were not sufficient qualified staff onsite to meet the required staff-to-child ratios.

An audio monitoring device shall be used in any area or room where children under one year of age are placed to sleep. On the day of the complaint visit the infant sleeping room did not have an audio device.
Rule states, in pertinent part, that the center director shall be responsible for the supervision of the planning and implementation of the center's program for children and the supervision of staff at the center. Upon review of staff schedules and other relevant documentation reviewed during the course of the complaint investigation, an employee at the center who is not listed as the center director is fulfilling these duties.
It was discovered during the complaint investigation that child care workers under the age of 18 were providing sole supervision and care to children on multiple occasions between the dates of 2/17/2026 and 2/27/2026. Rule states that no person under 18 years of age shall be left in sole charge of a child.
Staff D and Staff E were used to meet staff-to-child ratios and were providing sole supervision and care to a group of 6 toddler-age children on the day of the complaint visit and had not completed the entry-level training requirements needed for their positions.
If a center is in operation for more than 3 hours per day, it is required that each child is provided with daily outdoor activities except during inclement weather or when not advisable for health reasons. During the licensing visit, staff stated that they have not been bringing infants outdoors on a daily basis.
The attendance was not current and accurate on the day of the licensing visit when 22 children were signed in yet 27 children were counted in the building.
Rule states to apply lotions, powders or salves to a child during diapering only at the specific written direction of the child's parent or the child's physician. The infant room did not have written directions or authorization forms from parents to apply lotions and salves that were in the room.
When asked, multiple staff stated that emergency evacuation plans had not been practiced since March 2025, or longer.
Staff B is missing a certificate from The Registry documenting that they have met the educational qualifications for a teacher and has worked as a teacher for at least 6 months.
Center self reported that on 4/17/25 a teacher in the preschool room grabbed a child by the arm and wrists, carried him across the floor and slammed him down on the floor. Per rule, actions that may be psychologically, emotionally or physically painful, discomforting, dangerous or potentially injurious are prohibited.
Child #6 did not have a date enrolled recorded on the Enrollment History form on the day of the visit.
Child care workers shall be free of non-classroom duties when they are counted in meeting staff-to-child ratios. The teacher assigned to the 2 1/2 - 5 year old room was cleaning the bathroom and sweeping floors while the children were napping in a separate room unsupervised.
An inspection of the medical log book revealed the log hasn't been reviewed every 6 months as required by rule.
Child 2 & 4 were missing immunization records on the day of the monitoring visit
Child 6 and Child 7 did not have documentation of a Health History and Emergency form in their file on the day of the monitoring visit.
Each child under 2 years of age shall have an initial health examination not more than 6 months prior to nor later than 3 months after being admitted to the center, and a follow-up examination at least once every 6 months thereafter. Child #4 and Child #8 do not have documentation of a follow-up exam within the past 6 months.

Rule states that intervals between activities shall be planned to avoid keeping children assembled in large groups. It was observed that a group of toddler-aged children were placed in a small gated enclosure after mealtime, with no activities or toys available, for a period of over 5 minutes.
Staff D and Staff E were working as qualified teachers on the day of the monitoring visit and there was no documentation available indicating that these persons met the educational qualifications of a teacher position.

It was also discovered during the complaint investigation that 8 total child care workers (Staff A, B, C, D, E, G, H, and J) did not receive background checks prior to beginning work. Staff schedules reflected that these employees acted as child care workers and were providing care and supervision to children in the years 2025 and 2026.
The record of emergency drills was not available for review during the visit.

Both staff were providing sole supervision and care to children on the day of the complaint visit. It was also discovered during the complaint investigation that 8 total child care workers (Staff A, B, C, D, E, G, H, and J) did not receive background checks prior to beginning work. Staff schedules reflected that these employees acted as child care workers and were providing care and supervision to children in the years 2025 and 2026.
It was observed that child care workers were not promptly responding to a crying child's needs on several occasions.
There was not at least one qualified teacher supervising a group of 6 toddlers on the day of the complaint visit.
It was observed by the licensing specialist that 3 toddler-aged children were placed in a small gated enclosure after lunch. Physical restraint and placing children in confined spaces are prohibited by rule.
Documentation of completion of Abusive Head Trauma (AHT) training was not on file for Staff D and Staff E. AHT training is required to be completed before a child care worker begins to work with children under age 5.
The health, safety, and welfare of children was adversely affected on the day of the complaint visit and in the weeks prior. The center knowingly allowed child care workers under the age of 18 years old to provide sole supervision to groups of children on various days between 2/17/2026 and 2/27/2026. In addition, child care workers without a background check nor any teacher qualifications were providing supervision to a group of 6 children on the day of the complaint visit.
Staff records for Staff D and Staff E were not available to the licensing specialist on the day of the complaint visit.
Upon review of documentation available at the center, there was no record of days and hours worked and in which classroom when child care workers were included in staff-to-child ratios.
Staff D and Staff E were missing documentation of having received training within the past two years on child abuse and neglect laws, identification, and reporting.
On 3/4/2026, staff did not have sight and sound supervision of children when children in the Preschool Room left the classroom to use the hallway bathroom. It was observed by the licensing specialist that multiple preschool-aged children left the classroom, travelled across the hallway to use the bathroom, and were unsupervised for varying lengths of time. It was also observed that a child care worker stepped out of the Infant Room, leaving one awake child and three sleeping children under one year of age not within sight and/or sound supervision for a period of approximately 2 minutes.
There was not 35 square feet of usable square footage accessible to children when 3 toddler-aged children were placed in a small gated enclosure, with an approximate total square footage of 12.5 square feet.
It was observed that children under age 3 were provided with adult size spoons during lunch, and were having difficulty eating their meals. The licensing specialist requested age-appropriate utensils be provided for the children, which center staff then provided to the group of children.
It was observed that staff were not sitting at the table with children during lunch.
(Child Care Teacher - Entry-Level Training ) on 3/4/2026 : Staff D and Staff E were used to meet staff-to-child ratios and were providing sole supervision and care to a group of 6 toddler-age children on the day of the complaint visit and had not completed the entry-level training requirements needed for their positions.
(Supervision Of Children ) on 3/4/2026 : On 3/4/2026, staff did not have sight and sound supervision of children when children in the Preschool Room left the classroom to use the hallway bathroom. It was observed by the licensing specialist that multiple preschool-aged children left the classroom, travelled across the hallway to use the bathroom, and were unsupervised for varying lengths of time. It was also observed that a child care worker stepped out of the Infant Room, leaving one awake child and three sleeping children under one year of age not within sight and/or sound supervision for a period of approximately 2 minutes.

Staff A and F are working as qualified assistant teachers and are missing documentation of meeting the educational qualifications of the positions they hold.
The center did not have documentation of a high school diploma or it's equivalent for Staff B and D who hold a teacher position.
Staff A, B, C, D, and F did not have documentation of the yearly requirement of 15 hours of continuing education in 2024.
All reviewed Staff Records (Staff A, B, C, D, E, and F) were missing documentation of having received training within the past two years on child abuse and neglect laws, identification, and reporting.
Open packages of dry foods were observed in cupboards in the kitchen and on top of the refrigerator. Dry foods, if opened, are required to be stored in bags with zip-type closure or containers with a tight-fitting cover and should be labeled.
