Rule states that any staff who prepares meals shall document annual department approved training of at least 4 hours in kitchen sanitation, food handling or nutrition. Staff H that prepare meals for the center were missing documentation of this training.
Staff C was missing documentation of having maintained a current certificate of completion for infant and child cardiopulmonary resuscitation (CPR) and automated external defibrillator (AED) use from an agency approved by the Department.
Staff D has worked at the center for more than 6 months and were missing a certificate from The Registry documenting that the person has met the educational qualifications of the positions they hold and do not have other documentation of educational qualifications.
Each child under 2 years of age shall have an initial health examination not more than 6 months prior to nor later than 3 months after being admitted to the center, and a follow-up examination at least once every 6 months thereafter. Child #3 does not have documentation of a follow-up exam within the past 6 months.
Staff B, C, D, and H have worked at the center for more than 6 months and were missing documentation that the person has met the educational qualifications of the positions they hold.

The center self reported that on 8/15/24 at approximately 11:47am a child was not within sight and sound supervision of a child care worker when the worker stepped out of the classroom to go get puppies, which was unauthorized.
The center's child tracking procedure was not implemented as written when staff were not aware of the number, names and whereabouts of children in their care during a self-reported incident on 08/15/2024 when a child was left without supervision.
