A child care worker was observed wiping several childrens noses and did not wash her hands between/after wiping.
An infant aged 4 months was observed swaddled while sleeping, while swaddling is not recommended for children over 1 month and there was no permission on file from the parent.
There was not sufficient indoor play equipment to allow each child a choice of at least three activities involving equipment when all children are using equipment, particularly in the preschool.
Child #3 and #4 were missing emergency contact information for the child.

According to the center policy, diaper changes will be provided as a needed basis. During interviews with staff and the center director, a childs diaper is to be changed every two hours or every hour if a child has a rash and cream is applied. On 7/22/25, a child started to obtain a rash. Based on the childs transition report, a diaper change was conducted on 3:36pm, at which time, cream was applied. Staff failed to change the childs diaper one hour later.
On 9/3/25, after diapering, the changing table was not cleaned by staff using a two-step process. The changing table was not cleaned with a disinfectant, only with a multi-purpose cleaner.
In the preschool room, a childs hands were not washed with soap and running water after diapering.
A medication authorization observed on the premise does not include a begin and end date on the form.
(Child Guidance - Prohibited Actions ) on 8/13/2025 : Based on multiple staff interviews, a staff member was grabbing children by the arm and forcefully laying them down on their cots.
Staff D does not have documentation of a current child abuse and neglect training. The most recent training on file was taken February 2023.
Two infant and toddler intake forms were not in the room care was being provided.
Childrens Motrin and Childrens Tylenol being stored at the center are not labeled with the childs name.
On 1/29/25, the child tracking was not accurate in the preschool room; 7 children were in attendance but 8 children were marked in on the tracking sheet.
Attendance is not current and accurate on the day of the licensing visit. In the preschool room, there were 7 children in care but 8 children signed in.
Staff are not consistently documenting hours. On 1/29/25, two staff were signed into the DSP B room but only one staff member was present. And on 1/22/25, one staff signed into the preschool room but did not sign out.
Staff are not consistently documenting hours worked when used to meet staff-to-child ratio. On the day of the monitoring visit, three staff were signed into the Toddler room but only two staff were present. And in the DPSA room, two staff were signed in but only one was present.
The bathroom in the Toddler room had rust peeling away at the bottom of the door frame. The rust was sharp and accessible to children.
Four Infant and Toddler Intake forms were completed but were not on file in the same room care was being provided.
In the Infant B room, two bottles brought from home were labeled with the childs name but were not dated.
This licensing inspection recorded no violations or advisories.
The bathroom in the preschool/school age room are not maintained in a sanitary condition. Urine was observed in one of the toilets, stool was splattered in the toilet bowl and on the toilet seats, and several disposable gloves were laying on the floor.

Bedding in the preschool/school age room are not being maintained and stored in a clean and sanitary manner. Cots were not fully covered with a clean sheet/blanket.
There is no documentation that the medical log book in the school age room was reviewed within the last 6 months.
Several medications observed on the premise (Childrens Motrin, Childrens Tylenol, and Diazepam) do not have a medication authorization form signed and dated by the parent. A medication authorization for an Albuterol inhaler lacked a current authorization; the authorization expired July 2025.
Staff B, identified as the meal prep personnel, does not have the annual one hour training in kitchen sanitation, food handling, and nutrition.
Under 2 intake forms in the Infant A and Toddler room were not updated within the past 3 months.
