Infants & Toddlers were not allowed to form and follow their own pattern of sleeping and waking. After lunch the lights in the infant classroom were turned off while some children slept and some remained awake, content and playing with toys. A teacher then took the toys away from a child, the child was told it was time for nap, and the child was put in a crib at which time the child immediately began crying.
Minimum USDA snack requirements are not met when ice cream, Rice Krispie treats, Nilla wafer cookies and pudding are served as a snack component. In addition, per a review of the posted April snack menu the required two snack components are not always served.
Various bottles of commercial grade cleaning products (peroxide, bleach, toilet bowl and floor cleaner) labelled "keep out of reach of children" are accessible to children in an open metal storage cabinet that is located in a bathroom stall in the bathroom designated for the Preschool and 2's classroom. The doors to the cabinet are observed opened and a child is using the bathroom stall next to the cabinet during the visit.
Health history information on file for Child #1, who has a food allergy, is incomplete. Triggers have not been identified.
A teacher reported administering a child an asthma inhaler last week however the teacher did not record the administration of the inhaler in the medical log book.
Changes to the planned menu were not recorded. The lunch menu states "turkey/cheese, Doritos, green beans and cantaloupe" were served on the day of the monitoring visit however breaded chicken, hash browns, green beans and pineapple were observed to have been served for lunch.

Staff record information on file for Staff A is incomplete. The reason for Staff A having left a previous child care position has not been included.
Documentation of the days & hours worked, and in which classroom, when the person was included in the staff-to-child ratio is not always accurate. For example, a departure time was not recorded for Staff D on 4/2/25 in Infant Rm. 2 & 1's clipboard. A departure time was not recorded for Staff B in the 3's & 4's room when she is observed to have left the classroom for lunch. Staff D & E have combined the two infant classrooms & are observed working together in one infant classroom however the staff hours recorded document them working in separate infant classrooms.
Staff C, who has been employed since 11/6/24, has not obtained a certificate of completion for infant & child CPR with AED within 3 months after beginning to work with children in care.
Child tracking is inaccurate in the Preschool Classroom when a child who has left the classroom is still listed as present on the classroom tracking sheet.
