Based upon investigation, on or around March 2025, parents were not immediately notified when a child enrolled in care bumped their head on the playground equipment resulting in a bloody nose.

Based upon investigation, injuries to a child while enrolled in care were not documented within the medical log book.
Based upon observation, a staff member counted in staff to child ratio was not signed into the infant room. This was corrected during the visit.
Based upon observation and record review on during the week of January 16th, the toddler room exceeded staff to child ratio when five children under the age of 2 were in care from 7-757am.
