Diapers were not disposed of in a plastic lined, hands free, covered container as required when diapers had been thrown away in uncovered garbage cans in the bathrooms.
The gate to the playground had a gap of 9 inches between the bottom of the gate and the ground. In addition, there was a gap of 7 inches between the gate and the side of the fence.
Each fire extinguisher was not inspected each year by a qualified person when the last documented inspection was from August of 2023.
Several playground climbers that were at least 4 feet high did not have 9 inches of energy absorbing surface under them as required.
