Per self-report, the center failed to adhere to a procedure to ensure the whereabouts of children in care were known to assigned child care workers at all times, when a child under the age of one year old was left unattended in a classroom for approximately 4 or 5 minutes.
The center failed to use equipment in accordance to manufacturer's instructions when a 2 year old child was injured while using equipment identified for ages 5 to 7 years.
Center reported a child was injured as a result of jumping on an inflatable bounce surface (flexible seating chair).
Per observation, kitchen staff did not wash their hands prior to putting on gloves and handling food, during the monitoring visit.
