Food was not free from spoilage and safe for human consumption at the time of licensing visit. An open container of infant oatmeal cereal was not dated when opened; container states to use within 30 days of opening. Containers of yogurt were observed in the center refrigerator, dated best by December 21, 2025.
The files for Staff A and Staff C lacked documentation of an orientation being completed within staff's first week.
Staff C lacked Abusive Head Trauma Prevention training. Staff C has been employed since October 2025 and was present during licensing visit.
Staff B lacked current training in Cardiopulmonary Resuscitation (CPR). Staff B's last CPR training expired November 2025.
The two-year-old room did not have a qualified child care teacher at the time of licensing visit, both staff were assistant teacher qualified. Staff were adjusted during visit and a qualified teacher was moved into the classroom.

(Premises - Well Drained, Clean, In Good Repair ) on 9/17/2025 : The premises was not maintained in good repair when a cupboard in the toddler 3 room had a loose face board. A cupboard was missing a face board in the preschool room. An order was issued for this violation October 23, 2024. $50

Direct forfeitures were issued for this violation on July 31, 2024 and March 26, 2025.
On May 28, 2025 tracking was not utilized when two 2 year old children exited the gym via an exterior door, and were not found until people walking past the center saw the children crying by a gate. The children were outside alone for approximately 10 to 18 minutes. Staff were unaware the children exited the building. In addition, the tracking document used by staff on May 28, 2025 was not accurate when a review showed three children did not have departure times documented. A direct forfeiture was issued for this violation on July 31, 2024.
