The licensee shall ensure that all new staff and auxiliary staff, prior to working with children in the CBCCPP; and any new individual designated as the representative of the licensee shall submit a Records Check Authorization form to the Division and shall submit to fingerprinting at a location designated by the Division after having received the Fingerprinting Authorization Certificate by the Division.
One (1) substitute had not followed through with all pieces of their background clearance prior to this licensing visit. Staff A received the form to complete fingerprinting and the out of state record check form on 12/3/2025. On the same day the state of Florida returned the out of state form they received via email to the program and to the Division identifying the requesting agency needed to sign the form, not the Director. The following day the Child Development Division forwarded the same email chain to the program identifying "Your Florida background check form has been rejected (see below). These are supposed to be sent to our office for my signature. Please resend to me and I will sign and submit. Thank you." The program did not return the corrected form to the Division. In addition, Staff A was unable to provide a receipt or indicate the location and date fingerprints were taken to confirm compliance with that portion of the background check. Post visit, the program did send a fingerprint receipt showing that the substitute had been fingerprinted on 2/19/2026, the day after the licensing visit.
