A review of each staff member's BFIS Quality and Credential account and confirmation of the staff member that they had not completed the Vermont Orientation as required. They have been working in the program longer than one (1) month.
A review of staff BFIS Quality and Credential accounts and review of staff files confirmed five (5) staff members had not completed the required fifteen (15) hours of professional development as required for their positions.
A review of staff's BFIS quality and credential account and verbal confirmation from the program that three (3) staff had not renewed their CPR and First Aid certificate prior to the expiration date.
A staff member did not submit to fingerprinting prior to the expiration date of 12/19/2022. A record check was submitted on 2/8/2024 and the staff member had not completed the fingerprint process as of 6/24/2024. The staff member has worked part-time for the last few years during the summer camp. An annual assurance letter was issued to the program on 11/20/2022 reminding the program of this staff members fingerprint expiration date.

After a review of staff BFIS Quality and Credential accts and staff documents presented by the director, five staff have not completed a Division approved orientation training. All staff are in positions to be left alone with children and/or counted in staff to child ratio.

Staff BFIS Quality and Credential accounts were reviewed and three staff have not submitted qualification documentation within six months of beginning employment at the center.
After a review of staff BFIS Quality and Credential Accounts prior to the visit and staff documents presented by the director, five staff are not certified in pediatric first aid and/or infant and child CPR. Per staff start dates, all three staff have been working at the program longer than three months.
A review of staff BFIS Quality and Credential Accounts and staff documents provided by the director, five staff have not completed the Protecting Vermont's Children mandated reporter training or could not find certificate to be confirmed. All staff are in positions to be left alone with children.
