During a review of the provider's BFIS quality and credential account, and confirmation of the date trainings were completed, it was determined the provider did not complete nine (9) hours of the required fifteen (15) hours of professional development between 9/1/2021-9/1/2022.
The last date the fire drill was documented was 3/27/2023. The provider stated they completed fire drills but did not document them.
The fire extinguisher had an official tag stating the last date of inspection was 2/2021.
The children's files were audited, and the required immunization record was missing for two (2) children.
The children's files were audited, and the required well care exam was missing for two (2) children who have been in care for more than 45 days.
After a review of staff BFIS Quality and Credential Accounts prior to the visit and staff documents presented by the provider, a staff person was not certified in pediatric first aid and/or infant and child CPR. Per start date, the staff person has been available to work at the program as a substitute longer than three months and the provider confirmed that when working, the substitute is working in the absence of the provider.
