Upon review of the Associated Party list with the provider, it was determined that a household member had not completed the Background Clearing process as required. An authorization letter was issued on 7/24/24 for a household member and as of 5/26/25, this individual had not completed the the fingerprinting process.

A review of the provider BFIS Quality and Credential account showed that 12 hours of professional development were not completed for the period of 03/01/2024-03/01/2025. The provider could not find additional training certificates, but did send six (6) hours of training taken after this visit. It is required to complete 15 hours of professional development every year.
A household member present in the household did not submit to the fingerprinting process. The provider stated that the household member had not completed the fingerprinting process.
A review of the provider BFIS Quality and Credential account indicated missing training certification. The provider stated that not all training certifications had not been sent or added to staff BFIS Quality and Credential. The provider was able to show all documentation requested.

Upon review of the provider's BFIS Quality and Credential account and discussion with the provider, it was found that her educational documentation was missing in her account. The provider stated that she did not believe she had sent her information to Northern Lights.
