The FCCP shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.

During the visit, an individual was left alone with 6 children. A review of the BFIS account reflected that the individual was not approved to be left alone with children from the Division, as she had not completed the fingerprint supported background check. The provider stated that the individual had not completed the fingerprint supported background check.

7.2.4 · Not corrected

The FCCP shall ensure that at least one (1) responsible adult who has successfully completed and is currently certified in pediatric first aid and infant and child CPR is on the premises at all times children are present.

An individual left alone with children, counting in staff/child ratio was not currently certified in pediatric first aid and infant and child CPR as requested by this rule.

7.1.2.2 · Not corrected

The FCCP shall ensure that toys and other equipment used by children are sturdy and free from hazards such as but not limited to rough edges, pinch and crush points, splinters, exposed bolts, small loose pieces, areas that cause entrapment, and free from recall from the U.S. Consumer Product Safety Commission.

Upon arrival at the childcare, an infant about three (3) months old was asleep, covered by a blanket, in a Fisher-Price Rock 'n Play Sleeper located in the port-a-crib. The substitute stated that the child had been asleep for a while. Upon request of the licensor, the child was moved to a port-a-crib for safe sleep. The provider stated she was given the Rock 'n Play Sleeper about a year ago. After review of the U.S. Consumer Product Safety Commission website, it was found that the Fisher-Price Rock 'n Play was recalled on 4/12/19 and again on 1/9/23 due to infant fatalities.
