Evidence: Staff #1 had a TB screening that had been completed four months prior to employment, and not within the last 30 calendar days as required.
Evidence: On October 8, 2025, at 2:56 p.m. Staff #1 applied a non-prescription skin product on Child #1 after observing an irritation on the child?s arm. The Non-prescription authorization form the center had on file expired two months ago. The center did not have current written authorization from the parent to apply the skin product.

Evidence: When the three-year class utilize the restroom in the multi-purpose room after outside time, staff remain at the restroom and send children to hang up their coats in the three-year class. The three-year classroom is located outside of the multi-purpose room across from the Directors desk in the lobby. Interviews revealed that the Director is not always at their desk therefore, children from the three-year class were not within sight and sound supervision at all times.

Evidence: On 3/28/2025 at approximately 10:15am, Staff #1 was supervising 13 children, ranging in age from three-years-old to school age, in the multi-purpose room. Child #1, three-years-old, began poking other children in the group, Child #2, three-years-old, who was poked chased after Child #1 and pulled the child?s hair causing two braids to be pulled out of their scalp.
