On 5/1/2025, Staff A and Staff B transitioned children to the playground, which is located across an interior road in front of the school. During the transition, Staff A and B did not account for all children in their care. Child A was left unsupervised in the classroom, and both staff were unaware the child was missing.
The center did not request a search of the child abuse and neglect registry and a sex offender registry checks from State A, State B and State C for Agent #1, who is an applicant for licensure and does not work with children at the center.
Evidence: Staff #1's national fingerprint criminal background check expired in August 2023.

Evidence: 1. On 06/24/2024, at approximately 12:01pm, Staff #1 and Staff #2 left child #1 (13 months old) on the playground located across the interior road from the school while transitioning their group of students back to the center. 2. Staff #1 and Staff #2 confirmed they failed to perform a name-to-face check before exiting the playground, resulting in their unawareness that Child #1 had been left behind. 3.After crossing the interior road and entering the center's front door to conduct a headcount, Staff #2 noticed child #1 was missing and immediately returned to the playground to retrieve the child. 4. Based on video footage, Staff #2 found Child #1 on the enclosed playground at approximately 12:02pm. Child #1 was left alone and unsupervised on the playground for approximately one minute.
Evidence: There was no documentation that injury prevention procedures were updated at least annually based on documentation of injuries and a review of the activities and services.
Evidence: 1. There was no documentation of central registry results for Staff #2 (DOH: 3/13/2023). 2. There was no documentation of central registry results for Staff #3 (DOH: 5/24/2023). 3. There was no documentation of central registry results for Staff #4 (DOH: 4/24/2023).
Evidence: 1. On Wednesday, May 17, 2023, at approximately 4:00 pm, it was reported by Staff #2 that Child #1 (2-years-old) was seen kicking a group of wooden blocks in the classroom and running away. Staff #2 and Witness #1 both reported seeing Staff #1 grab Child #1 on the upper arm and pull them away from a table near the classroom door where Child #1 was standing, to sit in a chair at one of the classroom tables. Staff #2 reported hearing Staff #1 say "no" while forcefully pulling Child #1 to their seat. 2. Staff #2 stated that they spoke with Staff #1 after the incident and told them that "it was inappropriate and that they should not put their hands on the children and forcefully pull them." Staff #2 reported that Staff #1 apologized, stating, "I'm sorry, it won't happen again."
Evidence: Staff #1 (DOH: 9/27/2021), staff #3 (DOH: 8/6/2019), and staff #5 (DOH: 11/12/2019) record did not contain a central registry check results from a state they have listed as residing in within the previous five years.
