The center's ?Lost or missing children? policy states "It is the program?s strict policy that no child is ever left alone. It is the teacher?s responsibility to know where children in attendance are at all times. Children must be under sight and sound supervision at all times.? On 9/12/2022, at approximately 3:24 p.m., Staff 2 reportedly told leadership that they could not locate Child 1 (age 2 years) on the playground or in the classroom. The child could not be located for at least 15 minutes.

Evidence: 1. The record of Staff #1, hired on 10/8/21, contained documentation of fingerprints dated 10/18/21. 2. The record of Staff #3, hired on 12/23/21, contained documentation of fingerprints dated 1/13/22.
Evidence: 1. An incident report dated November 2, 2021, indicated that the parent was notified "upon present at 4:15pm." 2. During interviews, staff agreed the documented information on the incident was not correct and that the parent wasn't notified "upon present." 3. Administration stated the child's teacher had the incident report on a clipboard and left for the day without notifying the front desk that an injury had occurred. Administration stated "we have to work on notifying parents the same day."
