Based on record review and interview, two of two staff records did not contain required out of state background checks. Evidence: 1. Staff #1 (Date of hire-06/14/2024) confirmed that they have lived in the state of Pennsylvania(PA) within the past 5 years. Staff #1 did not have the completed PA criminal history and PA child abuse and neglect registry check that is required within 30 days from employment. 2. Staff #2 (Date of hire-08/16/2024) confirmed that they have lived in the state of New Hampshire(NH) within the past 5 years. Staff #2 did not have the completed NH criminal history search, NH child abuse and neglect search, or the NH sex offender search in their record. These searches are required before employment. 3. Staff #2 is a Provisional hire. Staff #2 was not supervised at all times by an individual with all required background checks completed within the last five years. 4. Staff #2 confirmed that they were left alone with children.
Evidence: 1. In the Infant classroom on June 20, 2024 at approximately 2:38pm, Staff #1 is observed standing in front of two highchairs with Child #1(10 months old) at their feet. Staff #1 grabs Child #1's right forearm and carries them approximately 3 feet and places them on the carpet. Child #1 is not crying. Child #1 crawls towards Staff #1 by the two highchairs in the classroom.

Evidence: 1. On 03/25/2024, at approximately 9:17am, Child #1(2 months) was placed in a bouncy chair by Staff #2. Child #1 was removed from the bouncy chair at approximately 10:43am. 2. On 03/25/2024, Child #1 did not receive a total of 30 minutes each day of tummy time to facilitate upper body strength and to address misshapen head concerns. 3. Staff #1 confirmed that Child #1 was placed on their stomach for approximately five minutes, but because their face turned red, stopped tummy time. Staff #1 confirmed that tummy time for Child #1 was not done for the rest of the time in care.
