1. On 10/23/2025, Infant classroom attendance sheets identified one staff member supervising five children from 7-8am. One staff member supervising ten infant children from 8-9am. Three staff members supervising thirteen infants from 9am to an undetermined time. Two staff members supervising ten infants from 3pm to an undetermined time. 2. On 10/27/2025, two staff were observed supervising nine children (youngest child four months old). The required ratio is one staff per four children.
Staff #1 and Staff #2 did not follow discipline policy based on the center's policies and procedures for appropriate behavior management and redirection.

Evidence: On 06/03/2024, staff S2 was observed on video attempting to wake C1 from a nap. S2 was observed lifting the long side of the cot and rolling C1 off the cot onto the floor. C1 reported that she was sleeping, and it was scary, and that the lady at the school woke her up, and that it hurt
Based on documentation and interview, the center did not ensure compliance with ratios of staff to children as required. Evidence: (Repeat Violation) On 03/27/2024, the licensing inspector entered the Infant I room at approximately 8:39 a.m. and reviewed the ?Face-To-Name Transition Sheet?. The Face-To-Name Transition Sheet documented S1 being in the classroom by herself between 7:22 a.m. and 7:43 a.m. with 10 children until S2 removed 4 children from the classroom at 7:43 a.m. The Face-To-Name Transition Sheet documented S1 being in the classroom alone again between 7:43 a.m. and 8:00 a.m. with between 6 and 11 children when S3 removed 7 children from the classroom. S1 acknowledged the room was out of ratio several times that morning as well as other times on other days. S4 reviewed the Face-To-Name Transition Sheet and acknowledged the Infant I room was out of compliance with the allowed ratio of 1 staff to 5 children due to the ratio waiver being granted.
On 2/13/2024, the licensing inspector entered the Infant I room at approximately 8:42 a.m. and observed 8 infants being cared for by 1 staff member (S1). The infant classroom is allowed a ratio of 1 staff to 5 children due to the ratio waiver being granted. S1 acknowledged the room was out of ratio when licensing inspector entered the room as well as other times on other days. The ?Face-To-Name Transition Sheet? documented S1 being in the classroom by herself between 8:14 a.m. and 8:44 with 7 children. A second staff (S3) entered the classroom at approximately 8:44 a.m. On 2/13/2024, the licensing inspector entered the Infant 2 room at approximately 8:47 a.m. and observed 8 children being cared for by 1 staff member (S2). The infant classroom is allowed a ratio of 1 staff to 5 children due to the ratio waiver being granted. S2 acknowledged the room was out of ratio at that time as well as other times on other days. The ?Face-To-Name Transition Sheet? documented S2 being in the classroom by herself between 8:16 a.m. and 8:57 a.m. with eight children. S3 entered the classroom at approximately 8:57 a.m. and removed five children from the classroom. The S4 was interviewed and reported that the center had multiple staff members call out due to illness the morning of 02/13/2024.

(Repeat Violation) Based on observation the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: A child, age 3, was observed by licensing inspector walking down the hallway by himself. He entered a hallway bathroom without staff providing actual sight and sound supervision.

1) Staff interviewed reported: A) That the center implemented a monthly fire evacuation drill at approximately 11:30 a.m. on 08/07/2023. After the drill was completed, the fire alarm in the building serving the school age children would not silence. B) The alarm continued to sound all day between approximately 11:30AM until approximately 5:00 p.m. C) That the children enrolled in the school age program remained on site with staff at the center for the day while the alarm continued to sound for approximately five(5) and one half (1/2) hours. D) That staff rotated the children between the outside playground and the inside of the building for lunch, snacks, water, and bathroom breaks with the alarm sounding between approximately 11:30 a.m. and 5:00 p.m. E) That the fire alarm sound was extremely loud in the building. 2) A parent interviewed reported that that their child after extended exposure to the fire alarm is complaining of ear pain requiring medical assessment. 3) On 08/07/2023 between approximately 11:30AM and 5:00PM and while the alarm continued to sound, staff had the children outside in between the times they were in the building for lunch, snacks, water, and bathroom breaks. The outside temperature ranged between 85 and 92 degrees.
Evidence: The records for S6, employed 02/28/2022 and S7, employed 08/15/2022 did not contain a certificate of showing completion of the 2023 annual health and safety update course.

Evidence: 1) The record for Staff #1, hired on 02/21/22, indicated the staff had resided in another state outside of Virginia within the last five years. The out-of-state sex offender registry check was obtained on 02/24/2022. The record did not contain an out-of-state criminal history record of information check from the state Staff #1 had resided. 2) The record for Staff #3, hired on 11/29/21, indicated the staff had resided in another state outside of Virginia within the last five years. The out-of-state sex offender registry check was obtained on 11/30/21. The out-of-state criminal history record of information check was obtained on 12/06/21. The out-of-state search for founded complaints of child abuse or neglect was requested on 12/01/21, but the results had not been received to date. 3) During interview, a member of management confirmed the required out-of-state background checks for Staff #1 and Staff #3 were not obtained within the required time frames. The out-of-state criminal history record information check and the sex offender registry check are required to be obtained prior to hire. The out-of-state search for founded complaints of child abuse or neglect is required to be requested within the first 30 days of being hired.
