1. Staff #1 confirmed that on 12/17/2024, Child #1 (age 3 years) was served food at lunch time, exposing the child to an allergen. 2. Child #1 was sitting at a table designated for children with food allergies. Staff #1 did not verify the list of allergens that was posted in the classroom before serving food to Child #1. 3. Child #1 suffered an allergic reaction, which included coughing, vomiting and difficulty breathing and was administered emergency medication and was transported to the local emergency hospital.
The written list of children's allergies in one classroom was not current. The list included a child who is no longer in the classroom.

Evidence: 1. The Center's policy for administering prescribed medication requires a current medication authorization signed by a physician. 2, An emergency medication for child #4 was onsite without a current medication authorization.
