A written agreement between the parent and the center shall be in each child?s record by the first day of attendance. The agreement shall be signed by the parent and include an authorization for emergency medical care, a statement that the center will notify the parent of illness, and the parent will pick up as soon possible and a statement that the parent will inform the center of any communicable disease in household.
Child #1, #3 and #5. who had all been attendance for more than two months, did not have a signed agreement with the two required statements.
