The sworn disclosure statement form used for staff A was dated in 2014 and not the current 2024 form. The up to date form was overdue by 6 months.
On 8/19/25 at approximately 3:55pm, Staff #1 roughly handled Child A. Observed evidence revealed that Child A was skipping past Staff #1 who was sitting in a chair. Staff #1 grabbed the upper arm of Child A and pulled the child over.
Staff A was hired as a lead teacher however the staff did not have the 24 hours of lead teacher training hours. This training was overdue by 17 months.
The immunization records for child A, B and C did not include a physician/ designees signature.
There was no central registry finding on file for Staff #1, who has been employed for over 2 months.
Evidence: Staff B's Sworn Disclosure Statement was dated 1/11/19.
Evidence: identified through staff interviews that around 1pm on Staff 1 observed Staff 2 pull child 1 up by their arm. Staff 1 noticed that Child 1 stopped using their arm, was favoring it, and was acting like it hurt them during the day, and reported the incident to the administration staff. Child went home, and parents noticied that the child was favoring their arm, causing them to take the child to urgent care where they were diagnosed with a dislocated elbow.
Evidence: Staff 1 and Staff 2 did not properly conduct name to face checks before leaving the classroom to take children outside, and left Child 1 in the classroom, unsupervised for approximately 6-15 minutes.
