The Religiously Exempt Child Day Center is required to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment. In the inside play area, 4 vents on the floor were not attached to the floor creating a tripping hazard.
