1) The Center?s ?Behavior Guidance Plan For Parents? states that ?We will offer alternatives to children engaged in undesirable behavior by offering a different toy, suggesting a new activity, engaging the child in an activity with a teacher or another child, or by suggesting independent play? and that ?the teacher helps the child to understand the logical consequences of his/her actions by removing an object, activity etc. that is causing the problem.? The ?Behavior Guidance Plan For Parents? also prohibits corporal punishment. On 6/27/25, Child #2 scribbled on the floor with a marker. Staff #2 and Parent #1 witnessed Staff #5 spank Child #2 before making Child #2 clean up the marker with Lysol spray and paper towels. Staff #5 then spanked Child #2 again. 2) The Center?s ?Child Supervision Record, Quick Guide? requires staff to ?Write the exact times for arrive (sic) and departure.? The Child Supervision Record for Classroom #2 dated 6/27/25 does not document the departure time for three children. The Child Supervision Record for the Classroom #6 dated 6/27/25 does not document the departure time for seven children. 3) The Center?s ?Child Supervision Record, Quick Guide? requires that when a staff member ?enters ratio they must sign in with their first and last name, noting the arrival time. When they leave, for any reason, including break or leaving for the day they must sign out with the corresponding time.? A) Two staff members did not sign out for breaks on the Child Supervision Record for Classroom #1 dated 6/27/25. B) One staff member did not sign out when she left for the day on the Child Supervision Record for Classroom #2 dated 6/27/25. A) Management did not document observations of any Name-to-Face and/or transition processes on the Child Supervision Records for Classrooms #1, #4, #5, and #6 dated 6/27/25. Evidence: 1) The Center?s ?Behavior Guidance Plan For Parents? states that ?if inappropriate behavior continues, parents will be asked to participate in an immediate parent/teacher conference.? There was no documentation of a parent/teacher conference in the record of Child #1. There were seven incident reports documenting physically aggressive and bullying behaviors toward other children and staff by Child #1 between 4/17/24 and 7/16/24. 2) The center?s ?Child Supervision Records (CSR) Standard Operating Procedure (SOP)? requires teachers to ?sign in and out of classroom CSRs at the exact time they arrive or leave.? The CSR for the school age classroom dated 7/15/24 documents no staff in the classroom between 12:30 p.m. and 1:25 p.m. A Name- to-Face Check completed at 12:00 pm documents 13 school age children in the classroom. A Name- to-Face Check completed at 2:00 pm documents 15 school age children in the classroom. No transitions were documented between 12:00 and 2:00. 3) The center?s ?Child Supervision Records (CSR) Standard Operating Procedure (SOP)? requires teachers to ?complete and document name-to-face checks on the CSR eight times a day? aside from transition times. The CSRs for the school age classroom dated 7/15/24 and 7/16/24 document six name-to-face checks. 4) The center?s ?Child Supervision Records (CSR) Standard Operating Procedure (SOP)? requires that ?Managers must observe at least two Name -to-Face Transitions per classroom daily and review (and sign!) every classroom?s CSR at least twice daily.? Evidence: According to reports and interviews, child, C1, age 3, was left alone on the playground on 11/14/2023 for approximately 4 minutes. The classroom teacher, S1 did not know the child walked behind her after she called his name during the transition from the playground to the to the classroom. C1 was left alone on the playground from approximately 11:30 a.m. to 11:34 a.m. At approximately 11:34 a.m.,the director observed the child on the playground from her office window. The incident report prepared by the center documented that C1 walked inside when his name was called but then turned around and walked behind S1 and walked back outside.
