On 09/25/2024, S1 lifted C1, (age 2), off the ground by one arm to have the child get off a rocking toy. The child was observed to be favoring her arm after this incident. The center was later notified that the child had been seen at the emergency room and had sustained a break in the wrist growth plate.

S1, employed for 5 months, did not have documentation of requesting a sexual offender check from one state. 2. S2, employed for more than 5 months, did not have documentation of requesting a central registry check from one state. 3. S2 reported that she and another staff were supervising the toddlers and 2-year-olds in the center?s inside playground. She stepped away briefly from the group to attend to another child needs. S2 did not realize that C1 had gone into the classroom out of actual sight and supervision of staff until the child?s mother came in to pick up the child and located the child.
