Staff 1 and Staff 2 failed to follow the center's supervision policy. As a result, Child 1 (age 3 years) was left alone for 45 minutes. The staff handbook says "Name to face protocol is followed with attendance tracking periodically to ensure each teacher is aware of and visually locating the designated children in their class throughout the day."

Based on observation and staff interview, the facility failed to clean and sanitize the diaper changing table as required after each use. Evidence: Staff used a disinfectant wipe to wipe down the diaper changing pad. A spray sanitizer was not used. After diapering, the pad should be cleaned with soap and water, sprayed with bleach water or another sanitizer, and allowed to air dry for a period of at least two minutes. Staff stated that she asked where the spray cleaner and sanitizer were located, but the wipes were readily available.
